Update on Targeted Case Management (TCM) and Supports Coordination Programs.
Public Announcement:
The Lycoming-Clinton Joinder Board has decided to end its Targeted Case Management (TCM) and Supports Coordination programs.
This decision was made because of funding changes at the State level that do not fit with our role as the bi-county MH/1D program. Because of this, the Joinder Board has decided to move these services to other provider agencies in the bi-county.
We want to reassure you that these services are not ending. Targeted case management and supports coordination services will continue to be available in Lycoming and Clinton Counties. What is changing is who will be providing the service.
Over the coming months we will be arranging for presentations by the new service providers so individuals can make an informed decision about which new service provider they want to work with.
The Lycoming-Clinton Joinder staff will continue to work with all TCM and SC consumers until the transfer occurs with the primary goal of making the transition as seamless as possible.
This decision does not impact any other mental health or intellectual disabilities service in the bi-county or the Joinder's role as the bi-county MH/ID office.
